1. Contact the EPRU Secretary with name, address, phone & email of primary team contact.
2. Complete the Permanent High School Application Form and mail it to the address on the form (wth attachments as requested on the form). Jerseys colors are requested at this time on the form
   (wait for approval before making any purchase).
3. Coach signs the "Coaches Code of Conduct" and mails it to the address on the form (can be mailed at the same time as the application).
4. Players and Coach complete and submit CIPP Enrollment Forms to USA Rugby.
5. Complete "Request for Insurance Certification" Form and submit to USA Rugby. (In order to get this Certification, the Club must also complete and submit the Club CIPP Enrollment Form).
6. Coaches must become familiar with the "Safety Precaution Recommendations" and "Making Rugby Safer" publications (and agree to follow any procedures recommended in same).
7. Club receives letter from the EPRU approving (or denying) request for membership and approving (or denying) jersey colors.
8. If approved, Club sends check for $100 (EPRU Dues) to EPRU, P.O. Box 393, Exton, PA 19341.

All teams must have a minimum USA Rugby Level I Certified Coach. If they are not certified at the time the application is submitted, they must attend the next available clinic. All returning teams must complete a HS Renewal Application Form and submit with dues payment plus a $10 Renewal Fee.
